    PURPOSE: The purpose of this study was to examine the surgical outcomes of double free muscle transfer (DFMT) performed in patients with complete brachial plexus injury (BPI). METHODS: We retrospectively analyzed the outcomes of DFMT for 12 patients with complete BPI who were followed up for more than 2 years after the final muscle transplantation. Their mean age was 29 years (range, 18-41). Three patients underwent contralateral C7 nerve root transfer before the DFMT. The range of motion (ROM) of the shoulder, elbow, and fingers was measured. Patient-reported outcome measures, including Disability of the Shoulder, Arm, and Hand (DASH) scores and visual analog scale (VAS) scores for pain, were also examined. RESULTS: The mean shoulder ROM against gravity was 22° ± 8° in abduction and 33° ± 5° in flexion. Seven patients underwent phrenic nerve (PhN) transfer to the suprascapular nerves, and five exhibited asymptomatic lung impairment on spirography more than 2 years after PhN transfer. The mean elbow ROM against gravity was 111° ± 9° in flexion and -32° ± 7° in extension. All patients obtained elbow flexion >90° against a 0.5-kg weight. All patients obtained touch sensation and two recognized warm and cold sensations in the affected palm. The mean total active motion of the affected fingers was 44° ± 11°. All patients exhibited hook function of the hands. The mean preoperative and postoperative DASH scores were 70.3 ± 13.4 and 51.8 ± 15.9, respectively. The mean pain VAS score was 28 ± 31 at the final follow-up. CONCLUSIONS: Double free muscle transfer provided patients with complete brachial plexus palsy with good elbow flexion and hand hook functions. TYPE OF STUDY/LEVEL OF EVIDENCE: Therapeutic IV.

    RATIONALE: The genomic alteration of cutaneous angiosarcoma (cAS) is complex. Treatment efficacy of immunotherapy for cAS remains controversial and prognosis remains poor. Herein, we report a case of cAS with programmed cell death 1, programmed cell death ligand-1, New York esophageal squamous cell carcinoma-1, and melanoma-associated antigen 4. PATIENT CONCERNS: A 69-year-old man presented with a chief complaint of left thumb pain, with a soft tissue mass in the palmar side of the thumb. He had no past medical history. Three months prior, the man experienced the pain while scuba diving. He visited a nearby clinic, and magnetic resonance imaging revealed a soft tissue tumor on the palmar side of the thumb. He was referred to our hospital and a marginal excisional biopsy was performed. DIAGNOSIS: Pathological findings revealed an angiosarcoma with high-flow serpentine vessels. INTERVENTIONS: An excision was performed from the base of the thumb to achieve a wide margin. OUTCOMES: One year after the treatment, the patient has not experienced recurrence, metastasis, or complications. LESSONS: Histopathology of the excised specimen was positive for programmed cell death 1, programmed cell death ligand-1, New York esophageal squamous cell carcinoma-1, and melanoma-associated antigen 4; their expression may be a therapeutic target for cAS. Combining immunotherapy with surgical treatment may be effective for cAS.

    Salmonella infection predominantly causes four clinical syndromes: enteric fever, gastroenteritis, bacteremia, and asymptomatic carrier state. Salmonella osteomyelitis is an extremely rare manifestation of salmonella infection except in children with hemoglobinopathies. Salmonella osteomyelitis has been reported to mostly affect the diaphysis of long bones and lumbar spine. Here, we describe a case of salmonella osteomyelitis of the right 6th rib in a 74-year-old woman who presented with breast pain, swelling, high fever and local heat. Her medical history showed myocardial infarction; namely, at the age of 71, the patient had undergone the drug-eluting stent placement in the left anterior descending artery. A computed tomography (CT) scan at the first visit to another hospital showed a mass in the chest that invaded the ribs. 18F-fluorodeoxyglucose-positron emission tomography-computed tomography imaging showed a lesion suspected to be a mammary malignant tumor. A needle biopsy revealed mesenchymal cells and suspected mammary sarcoma. However, the osteomyelitis of the rib was diagnosed when pyogenic tissue was observed during an open biopsy. The bacterial culture examination identified Salmonella enterica. Surgical drainage and antibiotic treatment were performed. Importantly, there was no evidence for any underlying disease that could lead to an immunocompromised status of the patient. To our knowledge, this is the first report of salmonella osteomyelitis of the rib presenting in an older female that required differentiation from a mammary tumor. Clinicians should consider rib osteomyelitis when they find swelling and local heat in the female's breast tissue and detect no cancerous tissue.

    Purpose: This study aimed to report the outcomes of patients with stage III Kienböck disease after treatment with a vascularized bone graft (VBG) to the lunate combined with capitate shortening osteotomy (CS) after a more than 10-year follow-up. Methods: A VBG to the lunate was combined with CS in 10 patients with stage III Kienböck disease (6 patients with stage IIIA and 4 with stage IIIB). We performed VBG, CS, and radial shortening osteotomy (RS) on 7 patients. Among them, 4 had undergone RS previously. The passive wrist extension angle and wrist flexion angle, grip strength (GS), carpal height ratio, Stahl index, visual analog scale of wrist pain, and Mayo modified wrist score were assessed before surgery and at the final follow-up. Results: The flexion angle decreased markedly after surgery, when GS increased in all 10 patients. Radiographic examinations revealed that the carpal height ratio decreased in 9 of 10 patients, whereas the Stahl index increased in 8 patients and remained unchanged in 2. The oldest 3 of 7 patients who underwent VBG, CS, and RS exhibited fusion of the proximal carpals except the pisiform. The mean visual analog scale decreased from 27.6 before surgery to 5.7 afterward. The Mayo modified wrist score improved in 9 patients after surgery and remained unchanged in one. Conclusions: In stage III Kienböck disease, VBG to the lunate combined with CS relieved wrist pain and increased GS and lunate height but was followed by severely restricted wrist motion. Fusion of the proximal carpals developed in 3 of 7 patients who received VBG with CS and RS. Type of study/level of evidence: Therapeutic Ⅳ.

    肘関節周囲外傷後に人工肘関節全置換術(TEA)を施行した高齢者5例(全例女性、平均73.8歳)の治療成績について検討した。対象は上腕骨顆上骨折3例、陳旧性肘関節脱臼1例、上腕骨通顆骨折1例で、全例とも半拘束型TEAを用いて再建した。術後関節可動域の平均は屈曲が117°、伸展が-13°、回外が90°、回内が71°と良好であった。術後に尺骨神経症状を2例に認めたが、不安定性・感染・皮膚障害・再手術例はなかった。

    Dupuytren's pathologic tissue usually involves the palmar skin and rarely infiltrates into the dorsal skin. In this study, we hypothesized that transplantation of a vascularized cutaneous flap harvested from the dorsum of the hand to the palmar skin, under which the pathologic cord existed might be useful for blocking reformation of the pathologic cords and for decreasing the recurrence rate. After removal of the pathologic tissue under the palmar skin, we harvested 2 types of skin islands nourished by the dorsal digital arterial network including the dorsal perforating arteries arising from the palmar digital arteries. The skin islands were transplanted into skin defects in the palm of the fingers and hand that had been created after correction of flexion contracture of the fingers. We performed this surgical maneuver on 24 fingers in 24 patients who exhibited severe flexion contracture because of Dupuytren disease. During the mean 40-month follow-up, only 1 patient experienced metacarpophalangeal joint flexion contracture ≥20 degrees in the operated finger. The recurrence rate was 4.2%, which was almost similar to that for a dermofasciectomy followed by a large full-thickness skin graft.

    Epidemiological studies have shown an association between hypertension and knee osteoarthritis (OA). The purpose of this study was to investigate whether activation of the renin–angiotensin system (RAS) can aggravate mechanical loading-induced knee OA in mice. Eight-week-old male Tsukuba hypertensive mice (THM) and C57BL/6 mice were divided into running and non-running groups. Mice in the running group were forced to run (25 m/min, 30 min/day, 5 days/week) on a treadmill. All mice in the four groups (n=10 in each group) were euthanized after 0, 2, 4, 6, or 8 weeks of running or natural breeding. Cartilage degeneration in the left knees was histologically evaluated using the modified Mankin score. Expression of Col X, MMP-13, angiotensin type 1 receptor (AT1R), and AT2R was examined immunohistochemically. To study the effects of stimulation of the AT1R in chondrocytes by mechanical loading and/or Angiotensin II (AngII) on transduction of intracellular signals, phosphorylation levels of JNK and Src were measured in bovine articular chondrocytes cultured in three-dimensional agarose scaffolds. After 4 weeks, the mean Mankin score for the lateral femoral condylar cartilage was significantly higher in the THM running group than in the C57BL/6 running group and non-running groups. AT1R and AT2R expression was not detected at 0 weeks in any group but was noted after 4 weeks in the THM running group. AT1R expression was also noted at 8 weeks in the C57BL/6 running group. The expression levels of AT1R, COL X, and MMP-13 in chondrocytes were significantly higher in the THM running group than in the control groups. Positive significant correlations were noted between the Mankin score and the rate of AT1R-immunopositive cells, between the rates of AT1R- and Col X-positive cells, and between the rates of AT1R- and AT2R-positive cells. The phosphorylation level of JNK was increased by cyclic compression loading or addition of AngII to the cultured chondrocytes and was reversed by pretreatment with an AT1R blocker. A synergistic effect on JNK phosphorylation was observed between compression loading and AngII addition. Transgene activation of renin and angiotensinogen aggravated mechanical load-induced knee OA in mice. These findings suggest that AT1R expression in chondrocytes is associated with early knee OA and plays a role in the progression of cartilage degeneration. The RAS may be a common molecular mechanism involved in the pathogenesis of hypertension and knee OA.

    Recent data suggest that the lectin-like oxidized low-density lipoprotein (ox-LDL) receptor-1 (LOX-1)/ox-LDL system may be involved in the pathogenesis of arthritis. We aimed to demonstrate the roles of the LOX-1/ox-LDL system in arthritis development by using LOX-1 knockout (KO) mice. Arthritis was induced in the right knees of C57Bl/6 wild-type (WT) and LOX-1 KO mice via zymosan injection. Saline was injected in the left knees. Arthritis development was evaluated using inflammatory cell infiltration, synovial hyperplasia, and cartilage degeneration scores at 1, 3, and 7 days after administration. LOX-1, ox-LDL, and matrix metalloproteinase-3 (MMP-3) expression in the synovial cells and chondrocytes was evaluated by immunohistochemistry. The LOX-1, ox-LDL, and MMP-3 expression levels in synovial cells were scored on a grading scale. The positive cell rate of LOX-1, ox-LDL, and MMP-3 in chondrocytes was measured. The correlation between the positive cell rate of LOX-1 or ox-LDL and the cartilage degeneration score was also examined. Inflammatory cell infiltration, synovial hyperplasia, and cartilage degeneration were significantly reduced in the LOX-1 KOmice with zymosan-induced arthritis (ZIA) compared to WT mice with ZIA. In the saline-injected knees, no apparent arthritic changes were observed. LOX-1 and ox-LDL expression in synovial cells and chondrocytes were detected in the knees of WT mice with ZIA. No LOX-1 and ox-LDL expression was detected in the knees of LOX-1 KOmice with ZIA or the saline-injected knees of both mice. MMP-3 expression in the synovial cells and chondrocytes was also detected in knees of both mice with ZIA, and was significantly less in the LOX-1 KO mice than in WT mice. The positive cell rate of LOX-1 or ox-LDL and the cartilage degeneration score showed a positive correlation. Our data show the involvement of the LOX-1/ox-LDL system in murine ZIA development. LOX-1-positive synovial cells and chondrocytes are potential therapeutic targets for arthritis prevention.

  • 人工膝関節置換術後感染予防におけるバンコマイシンの局所投与の至適用量の検討 関節液・血液薬物動態について

    人工膝関節置換術27例34膝を対象にバンコマイシン(VCM)を関節内に局所投与し、経時的に血液内・関節内の濃度を測定した。VCMを1g投与した14例15膝と、0.5g投与した13例19膝(片側9例11膝、両側4例8膝)に分け、VCMについて血液中は帰室時および術後1、3、7日に、関節液中は術後1、2、3、5、7日に測定した。その結果、血液中VCMは1g群と片側0.5g群では術後1日に最も上昇し、その後漸減した。両側0.5g群では帰室時に最も上昇し、術後1日に片側1g群とほぼ同等となった。腎障害等を生じる有害濃度20μg/mlを超えた症例は1例もなく、血液中VCMは術後7日に全例で消失した。一方、関節液中VCMは有効局所濃度10〜15μg/mlを超えたのは1g群では術後2日まで、0.5g群では術後1日までで、術後7日に全例で消失した。人工膝関節置換術後感染予防におけるVCM局所投与の至適用量は0.5gであると考えられた。

  • 重度手根管症候群に対する母指対立再建術 手術法による検討

    14歳男児。右腰部・鼠径部・臀部痛が出現し、歩行困難となったため、当院へ救急搬送された。造影CTでは異常所見は認められなかったが、血液検査でCRPの上昇を認めたため、化膿性脊椎炎等が疑われた。精査入院翌日の腰椎MRIでは明らかな化膿性脊椎炎や腸腰筋膿瘍は認められなかったが、その翌日の股関節MRIでは右腸骨窩に液体貯留を認めた。右腸骨窩膿瘍と診断され、CEZ投与を開始した。血液培養からはCEZの感受性を認めるMSSAが検出された。炎症反応は改善傾向にあったため、投与は継続された。治療開始18日目にはCRPが陰性化し、右腸骨窩の液体貯留や症状も消失した。CRP陰性化以降はCFPN-PIの内服を6週間継続した。治療終了後も症状の再燃は認めず、経過は良好である。

    エタネルセプト(ETN)治療が5年以上経過している関節リウマチ患者31例(男性7例、女性24例、平均年齢64±13歳)の背景因子を検討した。その結果、ETN投与期間は平均7.4年で、初回ETN投与量は25mg/週が13例、50mg/週が18例であった。最終ETN投与量は25mg/週未満が19例、25mg/週が9例、50mg/週が3例であった。ETNは最終時には28例で減量投与がなされ、ステロイド使用症例の減少とステロイド使用量の減量、更にMTX使用症例、DMARDsやNSAIDs併用症例の減少が確認された。期間中に3例が手術を受けていた。合併症は肺炎3例、足趾潰瘍1例であった。長期例の特徴をETN投与量、罹病期間、DAS-28-CRP、BMIで検討したが、関連する項目はみられなかった。
  • 重度手根管症候群に対する母指対立再建術の検討

    重度手根管症候群に対し施行した各種母指対立再建術の術後成績を手術時間,術後の握力,指腹ピンチ力および自覚的機能評価を用いて比較検討した.内訳はCamitz変法14例,Bunnell変法15例,Burkhalter法10例であった.すべての方法で握力および指腹ピンチ力は術前後で有意に改善を認め,特にピンチ力の改善が大きかった.術前後の改善率の比較では,握力は手術法間で有意差はなく,ピンチ力はBunnel変法で最も高かった.Quick DASH日手会版による自覚的機能評価は各手術法で有意差は認めなかった.Camitz法は手根管開放術の創をそのまま利用して行える簡便な方法であるが外転再建しかできないためピンチ力が弱いのが欠点である.一方Bunnel法とBurkhalter法は動的な再建術であり力強いピンチ力が再建できた.握力やピンチ力の改善には差を認めたがいずれの手術法でも自覚的機能評価は良好であった.(著者抄録)

    平成27年9月〜10月の人工膝関節置換術(TKA)11例を対象に、術後の鎮痛目的のカクテル注射内にバンコマイシン(VCM)を混和して関節内に注入し、術後7日までの血中VCM濃度および関節液中VCM濃度を経時的に測定した。帰室時・術後1日・3日・7日の血液中VCM濃度(μg/ml)はそれぞれ2.6・6.1・1.1・0.8(測定限界)であり、術後1日・2日・3日・5日・7日の関節液中VCM濃度(μg/ml)はそれぞれ77.9・13.3・3.9・1.2・測定限界であった。有害事象は認めなかった。TKAにおいてVCMの局所投与は感染予防だけでなく耐性菌発生の抑制に有効であると考えられた。

    症例1:63歳男。肝移植術後、骨髄異形成症候群、透析歴3年の免疫抑制状態であった。右肩関節の腫脹を主訴とし、MRIで肩甲上腕関節と肩峰下滑液包内の液体貯留、上腕骨頭の骨髄内浮腫を認めた。培養にて基質特異性拡張型βラクタマーゼ産生大腸菌を検出し、菌血症性化膿性肩関節炎と診断して肩関節鏡視下滑膜切除術を施行した。関節炎は鎮静化したが、基礎疾患の悪化で3ヵ月後に死亡した。症例2:73歳男。虚血性心疾患、脳梗塞、糖尿病の治療中であった。右肩関節痛を来たし、培養で大腸菌を検出し、MRIで肩関節から肩峰下腔に及ぶ関節液の貯留、腱板の断裂を認めた。菌血症性化膿性肩関節炎と診断し肩関節鏡視下滑膜切除術を施行し、術後1ヵ月に関節炎が再燃したため、再び同様の手術を施行した。術中に採取した関節液の細菌培養は陰性で、慢性滑膜炎の状態であった。術後に関節炎は鎮静化した。

    初回人工膝関節置換術において、ロピバカイン150mg+エピネフリン0.2mg+10%トラネキサム酸20mLのカクテルにフェンタニル100μgを加えて関節内投与した90膝(I群)と、術前12時間からフェンタニルパッチ4.2mgを6日間貼付し、術中は同カクテルを関節内投与した36膝(P群)とを比較検討した。安静時VAS(視覚的アナログ尺度)は、P群で帰室2時間後より術後14日まで有意に低下した。運動時VASはP群で術後1日目より14日まで有意に低下した。鎮痛剤使用量、歩行器歩行、T杖歩行自立日、入院日数は両群間に有意差を認めず、JOAスコアは両群とも有意に改善を認め、両群間で差を認めなかった。嘔気、せん妄の頻度は両群間で有意差を認めなかったが、頻回嘔吐、せん妄により2例がフェンタニルパッチを途中離脱した。満足度はP群の方が良いが18例、同等が1例、I群の方が良いが1例であった。

    SL-PLUS MIA stem(M)は、SL-PLUS standard stem(S)の肩部の突出を減らした形状のステムで、大転子部の骨温存が可能となり、術中骨折リスクを低減させる。標記について明らかにするため、人工股関節全置換術および人工骨頭置換術を施行した47例(男性2例、女性45例、55〜95歳)を、Sを使用した36例(S群)とMを使用した11例(M群)に分け検討した。両群とも遠位のゾーン3、4、5では骨密度が維持されていたのに対し、近位のゾーン1、7では骨密度は低下傾向にあった。S群ではどのゾーンにおいても術後4ヵ月より、術後12ヵ月の方が骨密度は低下傾向にあったのに対し、M群ではどのゾーンにおいても術後4ヵ月より術後12ヵ月の方が骨密度の回復傾向を認めた。レントゲンでは全ての評価項目において両群間で有意差を認めなかった。

    74歳男。排尿・排便困難を主訴とした。約3ヵ月前に転倒し腰痛が出現し、前医でL1椎体圧迫骨折の診断で経過観察されていた。X線でL1椎体にcleftを認め、機能写で椎体の異常可動性を認めた。CTでも同様にL1椎体内にcleftを認め、椎体後壁損傷で脊柱管内に軽度の突出があり、椎体前壁の欠損も認めた。MRIでL1椎体内にT1強調像で広範に低信号、T2強調像で椎体cleftに高信号域を認め、脊柱管狭窄の程度は軽度であった。泌尿器科で尿路系の異常は指摘されず、L1椎体の異常可動性を原因とする神経因性膀胱と診断した。椎体形成術単独で治療を行い、術後に膀胱直腸障害が改善した。また、入院時に行っていた導尿も不要となった。

    23歳女。主訴は右手掌部の疼痛と母指の易疲労感であった。右手掌部において手掌皮膚線の橈側に拍動性の索状物を触知し、同部に圧痛を認めた。パワードップラー検査では母指球筋表層を走行する動脈を確認し、血管造影検査では皮下を走行する橈骨動脈浅掌枝が確認できた。MRIアンギオグラフィーでは橈骨動脈浅掌枝にくびれを認め、通常よりも表層に走行する橈骨動脈浅掌枝が正中神経障害を生じていると推測した。保存的療法が無効であったため、手術を施行した。伝達麻酔下に手掌皮膚線直上を切開すると、屈筋支帯の遠位で2本の正中神経運動枝の間を橈骨動脈浅掌枝が貫通していたため、橈骨動脈浅掌枝を切断し正中神経運動枝の下層に走行を変え再度吻合した。術前に認めた手掌部の疼痛・労作時脱力感はすみやかに消失し、術後2ヵ月で元職に復帰した。

    人工膝関節置換術後鎮痛におけるフェンタニル関節内投与と経皮的投与について比較検討した。人工膝関節置換術後鎮痛として全身麻酔下持続大腿神経ブロック(FNB)を併用した86膝(I群)と外来でトラマドール内服導入後にフェンタニルパッチを貼付した24膝(P群)を対象とした。安静時VASはP群で帰室時より術後14日まで有意に低下し、1ヵ月では有意差を認めず、運動時VASはP群で術後2日目より14日まで有意に低下し、1ヵ月では有意差を認めなかった。鎮痛剤使用量、歩行器歩行、T杖歩行自立日、入院日数、臨床評価等は有意差を認めず、嘔気はP群で22膝、I群7膝、嘔吐はそれぞれ21膝、6膝、せん妄は2膝、2膝で両群間に有意差は認めなかった。両側2期的症例が19例含まれ、頻回嘔吐、せん妄により2例がパッチを中途離脱し、満足度はP群の方が良いが15例、同等が1例、I群の方が良いが1例であった。

    当科ではDupuytren拘縮に対する治療方針として、30°以上の屈曲拘縮がありADL障害を認める患者には手術治療を施行している。手術方法は、拘縮が多数指に及ぶ症例には可能な限り手掌腱膜の全切除を行い、1指罹患例には部分切除を行っている。今回、これまでに手術を行った50例の成績を報告した。男性49例、女性1例、年齢16〜76歳(平均62歳)、1指罹患例が21例、2指罹患が22例、3指罹患が7例であった。両側罹患例が32例あり、このうち両側に手術を行ったのは5例であった。罹患指は中指10指、環指44指、小指46指であった。日手会Quick DASHの機能障害・症状スコアで評価した臨床成績は術後3ヵ月時が平均17点(9〜36点)、術後6ヵ月時が平均7点(0〜30点)であり、経過とともに改善した。術前後の屈曲拘縮角度を罹患指別・関節別にみると、中指のMP関節は術前平均21°から術後0°に改善し、PIP関節は術前平均5°から術後0°、環指のMP関節は術前平均15°から術後2°、PIP関節は術前平均25°から術後1°、小指のMP関節は術前平均29°から術後0°、PIP関節は術前平均44°から術後10°に改善した。

    RA手指障害に対しSwanson MP人工指関節置換術を施行した14例46関節の短期成績(術後平均4年8ヵ月)を検討した。検討項目は、MP関節の可動域の推移、握力の変化、日本手の外科学会quick DASHによるADLの評価、X線変化(尺側偏位、インプラント周囲の変化)とした。その結果、手指および母指MP関節可動域は術前後で総可動域に有意な変化はなく、伸展側に移動していた。握力も術前に比べ改善を認めなかった。ADL評価では、掌側脱臼による屈曲拘縮や尺側偏位のため術前ではサイドピンチでしかつまみ動作ができず、把持動作も困難であったが、術後では指先部でのピンチや把持動作が可能となった。X線学的評価では、グロメット非使用例12関節のうち術後インプラントの緩みを1例に、破損を2例に認めた。一方、グロメット使用例34関節では術後インプラントの緩みは認めなかったが、グロメットの沈下を12関節、インプラント辺縁の骨硬化像を21関節に認めた。

    41歳男。交通事故により多発外傷を負い、脳挫傷・くも膜下血腫・顔面骨折に対して約1ヵ月間入院治療を受けた。退院後、近医でリハビリを受けていたところ、右手関節に疼痛・腫脹が出現、手指の知覚鈍麻も自覚し、月状骨脱臼と診断され当科に紹介された。問診と諸検査の結果から、受傷後1年経過した月状骨の掌側脱臼で、手関節の掌背屈制限と正中神経麻痺をきたしていると考えた。治療法の選択にあたっては患者の職業(手の使用が多い労作業)を考慮し、早期仕事復帰を目的として近位手根列切除術を行い、手関節可動域の改善、除痛、痺れの軽減が得られた。

        Total elbow arthroplasty (TEA) is going to be widely indicated for not only rheumatoid elbow but also unstable elbow, comminuted fracture of distal end of humerus and ankylotic elbow. We reported the case of a patient with rheumatoid elbow with post-traumatic varus deformity who had undergone TEA.
        A sixty-year old man who works as a lunch provider admitted our hospital for deformity and instability of the elbow.He had received DMARD and predonisorone for RA, which failed to control his disease activity. Thus he was started on anti-TNF alfa therapy, which reduced poly-arthralgia, however his symptoms in the elbow did not improve. Physical examination showed limitation of range of motion (ROM) and painful instability at his elbow. Radiographic examination revealed thirty degrees of varus deformity and dysplastic change of humeral trochlea. Semi-constrained TEA was selected for his unstable RA elbow, because of massive bone defect of the trochlea and loss of ligamentous structure. Two days after the operation, ROM exercise was started using CPM with flexion blocking over 90 degrees.Postoperatively, ROM (extension/flexion) improved from -30/90; preoperative to -30/120; post-operative,and pain and instability disappeared.The JOA score improved from 39 points preoperatively to 91 points postoperatively, and he returned to former job.
        It was thought that TEA was a good procedure for unstable rheumatoid elbow due to severe bone defects or lack of ligamentous stability. Careful follow-up examinations are necessary to prevent implant failures including breakage, aseptic loosening and penetration of humeral or ulnar cortex.

    A case of dorsal fracture-dislocation of the proximal interphalangeal joint in which the volar plate had been pulled from its distal attachment without bony or cartilaginous attachments and the lip of the volar plate had also been detached separately and was obstructing full flexion of the joint is presented. A tentative mechanism of causation of this unusual variant of a common injury is suggested.

    A 5-year-old boy was brought to our hospital with a painful left hip joint after being struck by a car. The physical examinations showed the left hip to be flexed, adducted and internally rotated with an apparent shortening of the left limb. No neurovascular deficit was found. Plain antero-posterior radiographs revealed a posterior dislocation of the left hip without any fractures of the acetabulum. The hip was reduced using a closed method under conscious sedation. After the reduction procedures, the hip joint was expected to recover a normal range of motion. Four weeks after procedure, magnetic resonance imaging revealed no ischemic changes in the femoral head. One year after the injury, the patient recovered the motion in the left hip without complaints related to the injury.

    A 54-year-old woman complained of a painful, swollen, and clicking left wrist for 1 year. She had an 8-year history of systemic lupus erythematosus (SLE), treated with oral prednisolone. Flexion and extension of all fingers were difficult to initiate on wrist extension, and movement was accompanied by a palpable click at the wrist. Magnetic resonance imaging (MRI) and tenography revealed the expanded sheath of the flexor tendons and well-defined round free bodies known as rice bodies. Synovectomy and excision of rice bodies resulted in complete disappearance of swelling and triggering of the wrist.

    We have reported the involvement of protein kinase C (PKC) in the development of osteoarthritis in vivo. In the current study, we examined the effect of mechanical stress on chondrocyte metabolism and the activity of PKC in vitro. Low frequency and magnitude of cyclic tensile stretch loaded on chondrocytes increased proteoglycan synthesis. However, high frequency and magnitude of stress decreased proteoglycan synthesis, and under this condition DNA synthesis was enhanced. PKC inhibitor and pretreatment with phorbol ester reversed the stress-inhibited proteoglycan synthesis. The PKC activity was reduced. We propose that PKC is involved in stress-mediated inhibition of proteoglycan synthesis.

We previously demonstrated the involvement of protein kinase C (PKC) in the development of osteoarthritis in vitro, In this study, we examined the effect of mechanical stress on chondrocyte metabolism and the activity of PKC in vitro, Low frequency and magnitude of cyclic tensile stretch loaded on chondrocytes increased proteoglycan synthesis. However, high frequency and magnitude of stress decreased its synthesis, In this condition, activity of PKC was reduced.     Gene therapy offers a novel approach to treating human joint diseases such as rheumatoid arthritis. In the present study, we have used the retrovirus, MFG-IRAP, to transfer the human IL-1 receptor antagonist protein (IRAP) gene to rabbits' knees and have assessed its impact on inflammatory and chondrodestructive aspects of the acute phase of antigen-induced arthritis in these joints, Surprisingly, intra-articular expression of IRAP was three- to fivefold higher in arthritic knees than in nonarthritic knees, accumulating to levels of over 20 ng/knee in the highest expressing joints. This level of expression produced a marked chondroprotective effect but a milder anti-inflammatory one. Both the increased cartilage matrix catabolism and the inhibition of matrix synthesis that occur in antigen-induced arthritis were abrogated in the presence of the IRAP gene; the latter effect was particularly strong. Of the indices of inflammation that were examined, only leukocyte influx into the joint space was inhibited, and this effect declined with time. Concentrations of rabbit IL-1 were reduced by the IRAP gene, suggesting inhibition of an autocrine induction loop. These data demonstrate that the course of arthritic disease in the rabbit knee can be altered by genetic manipulation, thus encouraging the further development of gene treatments for human joint diseases.

    To determine the role of nitric oxide (NO) in the inhibition of aggrecan synthesis, we measured levels of NO produced by bovine chondrocytes from different layers of articular cartilage in the presence of interleukin-1 (IL-1). Chondrocytes from the superficial layer showed a large increase in NO synthesis in response to IL-1. Although chondrocytes from the deep layer also produced NO in response to IL-1, the amount was less than that from the superficial layer. Enhanced NO production evoked by IL-1 was accompanied by a significant inhibition of aggrecan synthesis. These data suggest that chondrocytes in both superficial and deep layer of articular cartilage inhibit aggrecan synthesis with IL-1 via NO production. In addition, superficial layer cells respond to lower amounts of IL-1 with respect to NO-production and inhibition of proteoglycan synthesis.

    Keratan sulfate (KS) is one of the major glycosaminoglycans in cartilage matrix proteoglycan. We find that exogenously added KS totally blocks keratanase sensitive glycosaminoglycan release measured by radiolabeled ligand. We also find that KS release is potentially inhibited by its own presence as measured by ELISA. When KS is digested with keratanase before its addition to the medium, its inhibitory effect on KS secretion is partially blocked. Exogenously added KS promotes the accumulation of the KS epitope in the cell layer.     Introduction : To repair the articular cartilage defect, cartilage explant is useful method. The key leading to the success of this regenerative medicine is to regulate the mechanical stress loaded on articular cartilage explant. Intraarticular hyaluronan (HA) injection is an effective treatment for cartilage degeneration. The purpose of this study is to determine the effect of mechanical stress loaded on the cartilage explant and possible usefulness of HA in this system. Methods : Mechanical compression was loaded on the bovine cartilage using the Biopress system. Proteoglycan (PG) and reactive oxygen species (ROS) synthesis were measured with [^<35>S] incorporation and fluorescent dye, respectively. Accumulation of peroxynitrite was determined with western blotting using nitrotyrosine antibody. Results : Mechanical compression inhibited PG synthesis and enhanced ROS. Externally added HA reversed stress-inhibited PG synthesis and attenuated ROS synthesis. HA also significantly decreased the expression of nitrotyrosine. Conclusions : Mechanical stress is essential factor to degradate the explanted cartilage and ROS play an important role. HA neutralized stress-enhanced ROS synthesis and resulted in the reverse of PG synthesis. These data suggest that HA plays anabolic effect as an antioxidant.

    Osteoarthrosis, a common pathway to joint deterioration, results when a physiological imbalance exists between mechanical stress on the joint tissues and the ability of those tissues to withstand that stress. Although mechanical stress is known to be an essential factor in the regulation of cartilage metabolism, the precise mechanisms involved have not yet been determined. We have previously demonstrated that reactive oxygen species (ROS) are involved in cartilage degradation. Decreased size of hyaluronan (HA), the major macromolecule in synovial fluid and imparts viscosity of synovial fluid, is reported in patients of arthritis. Purpose of this study is to determine the alteration in molecular weight range of HA with mechanical deformation loaded on the chondrocytes and the involvement of ROS in this action. We found that ROS depolymerized HA molecules. Cyclic tensile stretch depolymerized HA and induced ROS. Superoxide dismutase inhibited not only ROS induction but also HA depolymerization caused by the mechanical stress. These data suggest that ROS play an important role in mechanical stress-induced HA depolymerization.

    Many tissues or organs including bone and joint adopted their morphology under the influence of the environmental mechanical loading. The purpose of this study is to solve this mysterious process and apply it for therapeutic aspect as tissue engineering. We introduce the distraction osteogenesis, which afford to correct the deformity and lengthen the limb. Although mechanical stretch on bone, tendon and vessel is possible to apply, lag period in the bone maturation is necessary. To improve this system, we investigate the role of reactive oxygen species(ROS) under the influence of mechanical stress. We used in vivo model of leg-lengthening *and found the characteristic morphology, such as the invasion of endothelial cells and apoptosis of mesenchymal cells. We first focused on the distribution of apoptosis of the osteoblast in this model. Recent observations revealed the significant role of ROS in ischemia-reperfusion induced cell apoptosis. We then examine the effect of mechanical stress on generation of the ROS and apoptosis of osteoblast. Cyclic tensile stretch loaded oh these cells clearly enhanced ROS synthesis in a time-and magnitude-dependent fashion. The disruption of micro filaments abolished the stress-induced ROS synthesis. Rotenone, an inhibitor of the mitochondrial electron transport chain, enhanced stress-induced ROS synthesis. These data suggest that actin filament and mitochondria are involved in this action.

    Osteoarthrosis, a common pathway to joint deterioration, results when a physiological imbalance exists between mechanical stress on the joint tissues and the ability of those tissues to withstand that stress. Although mechanical stress is known to be an essential factor in the regulation of cartilage metabolism, the precise mechanisms involved have not yet been determined. To examine, at the cellular level, the underlying mechanisms that lead to cartilage degradation, we introduced a computerized, pressure-operated device (Flexercell Strain Unit) to induce deformation of chondrocytes. We found that while a low frequency and magnitude of cyclic tensile stretch applied to chondrocytes increased proteoglycan (PG) synthesis, a high frequency and magnitude of stress decreased its synthesis. Nitric oxide (NO) has been implicated in the pathogenesis of OA. Enhanced PG and NO synthesis were evident when cyclic tensile stretch was applied to chondrocytes. When NO production was inhibited, PG synthesis was enhanced further. These data suggest that stress-mediated alteration in PG synthesis was modulated by NO production.

    平成10年に引き続き,軟骨代謝において最近注目されている一酸化窒素(NO)の軟骨細胞に対する作用を3次元培養法を用いて行った.NO合成阻害剤の存在下では培養液中のNO量はコントロールの1/3に減少した.培養液中のNOを消去することにより軟骨細胞周囲の基質(cell asociated matrix)と細胞外の基質(further removed matrix)のプロテオグリカン量は減少した.軟骨細胞のDNA合成能およびグリコサミノグリカン合成能は有意に抑制された.更に,ヒアルロン酸のレセプターと考えられているCD44の発現を免疫組織染色法で検討したところCD44の著明な発現低下を認めた.以上のことより恒常的に存在するNOが軟骨細胞の機能維持に必要なことが示唆された. 次にこのNOの作用が軟骨細胞のアポトーシスに関与しているかを検討するために当科で購入した共焦点レーザー顕微鏡を用いて検鏡可能なAnnexinV法を用いてアポトーシスの検出を行う予定にしている.軟骨細胞の単層培養法でアポトーシスの発現が確認できた過酸化水素を添加して3次元培養法でもアポトーシスの発現が確認できるかを現在検討中である.この方法で3次元培養法でのアポトーシスの検出が可能であれば今後,NOのみならずサイトカインや他のガスメディエーターを用いた検討も予定している.

    Although arthrosis, such as rheumatoid arthritis, is one of the popular pathway of joint deterioration, the underlying mechanism of cartilage degradation has not been clarified. We reported the significance of nitric oxide (NO) and superoxide (SO) in the interleukin-i (IL-1) mediated cartilage degradation. In this series of experiment, we have focused on the mechanism of IL-1-inhibited proteoglycan (PG) synthesis. SIN-1, which leads to simultaneous generation of both NO and su peroxide inhibited PG synthesis. Both superoxide dismutase and hemoglobin reversed SIN-1-inhibited PG synthesis, indicating the simultaneous generation of superoxide is essential to inhibit PG synthesis. IL-1 induced peroxynitrite in articular chondrocytes and addition of peroxynitrite resulted in inhibiting PG synthesis, thereby suggesting that the concurrent generation of superoxide anion and NO is required for the action of IL-1 to inhibit PG synthesis. Peroxynitrite is a candidate for this underlying mechanism. To examine the role of gas mediator in vivo, transplantation of cartilage was carried out. We made full thickness cartilage defect in the rabbit knee joint (defect model). When perichondrium was transplanted in the defect, degradation of surrounding articular cartilage was inhibited (repair model). The levels of superoxide and NO in the surrounding cartilage of the defect model were significantly higher than the repair model. These data suggest that gas mediator plays an important role in the cartilage degradation.

    Recent observations clearly indicate the significant role of free radical family in the physiological or pathological condition. However, there have been no report indicating the involvement of these molecules in the development of arthrosis. We previously reported that superoxide desmutase inhibited interleukin (IL-1) -mediated cartilage degradation. In this project, we first develop the system to measure the levels of superoxide anion and found that IL-1 induced large amount of superoxide anion from articular cartilage. We also found the involvement of nitric oxide (NO) in IL-1-inhibited proteoglycan. These results are mainly concerning about the involvement of these gas mediators in the development of rheumatoid arthritis. We further demonstrated the increased levels of these molecules in the articular cartilage in experimentally induced osteoarthritis in vivo. Taken together, it is clear that these gas mediators play an crucial role in the disease process of osteoarthrosis and regulation of these molecules could lead to the prevention of disease process of arthrosis. We demonstrated that hyaluronic acid, which is widely used for the treatment of arthrosis clinically, had some regulatory effect on IL-1-induced cartilage degradation. We believe that these information provide us new strategy for the pharmacological treatment of arthrosis.

    Gradual distraction of osteotomized bone produces zonular callus tissue between each fragment. This callus tissue composed of matured and primitive bony trabecular layrs and a central mesenchymal cell layr, and proved to be a good experimental model for surveying the bone metabolism. Oncogene c-fos, osteonectin, osteocalcin and type I II III collagens were detected in this callus tissue made in rabbit tibiae and rat femora by means of northem hybrization, in situ hybridization, and immunohistochemically. c-fos was observed in the mesenchymal cells adjacent to the primitive bone, and suggested to in itiate the osteoblastic cell differentiation. Osteonectin was observed in the osteoblasts on the growing trabeculae and suggested to initiate bone matrix formation. Osteocalcin was suggested to control the maturation of bony tissue. Type III collagen was observed in the cells and matrix collagen fibers in the mesenchymal cell layr and appeared to initiate the mesencymal cell differentiation. Type I collagen seemed to distribute similarly to osteonectin and suggested to relate to the matrix formation of osteoblasts
