    本学出身の研修医が、本学以外の医療機関でどのように評価されているかを調査した。方法としては、本学の卒業生が研修する医療機関にアンケートを郵送し、臨床研修センター長もしくは研修担当の医師から回答いただき、その内容を分析した。卒業時までに修得すべき本学のアウトカム10項目のうち、「コミュニケーション能力」については、回答を寄せた27病院のうち21病院で、本学出身の研修医がよくできていると評価された。また、「チーム医療」に関しても「チームに溶け込む」といった表現で代表されるように、評価が高かった。一方、不十分であると評価された項目で最も頻度が高かったのは「国際化に対応できる教養と英語力」であり、7病院がとりあげていた。記述回答をテキストマイニングの手法で感性分析したところ、本学の全般的なイメージについては本学出身の研修医や医師に関する印象をあげた記載が多かった。内容としては、好感がもてる、元気、明るいというポジティブな表現が多かった。ネガティブな表現は少なかったが、本学出身者が学術活動に消極的という回答が該当した。これらの結果は今後の本学の教育プログラムを改善していく上で、貴重な資料になると考えられる。(著者抄録)

    A questionnaire survey was conducted to determine the current status of home medical care (HMC) waste collection by nurses for individual items and whether the collection rate differs with city size. The questionnaire was mailed to 1,022 nursing offices, of which 645 offices provided valid responses. Offices were classified into three groups according to the local population size. Responses indicated that used syringes and needles, except for pen-type self-injection needles, were collected by >50% of HMC nurses. On the other hand, enteral nutrients, nutritional adjustment diet vessels, feeding tubes, portable injectors, ventilator masks, endotracheal suction catheters, tracheal cannulas, continuous ambulatory peritoneal dialysis (CAPD) bags, and urinary catheters and bags had a low collection rate in comparison with syringes or needles. The collection percentage of used syringes, needles, and infusion needles (p < 0.05) and that for infusion bags, tubes, and catheters (p < 0.001) differed among the three population groups. Although municipalities are responsible for collect all HMC waste, sharp or infectious items should be collected by HMC doctors or nurses. On the contrary, nonhazardous HMC waste should be collected by municipalities.Implications: This paper illustrates the status of waste collection of individual home medical care (HMC) items by HMC nurses. Infectious waste, such as needles and syringes, is collected by nurses. The collection rate of infectious waste by nurses in large cities was low compared with that in small cities. Although municipalities are responsible for collecting all HMC waste, sharp or infectious items should be collected by HMC doctors or nurses, whereas nonhazardous HMC waste should be collected by the municipalities.

    The Japanese Population-based Osteoporosis (JPOS) Cohort Study was launched in 1996 to produce a reference database of areal bone mineral density (aBMD) by dual energy X-ray absorptiometry (DXA) and bone turnover markers in the Japanese female population and to determine risk factors for osteoporotic fractures. At baseline, 3984 women aged 15 to 79 years were randomly selected to provide representative bone status data and aBMD values for the diagnosis of osteoporosis. Follow-up surveys were conducted in 1999, 2002, 2006 and 2011/12 to determine changes in aBMD and identify incident morphometry-confirmed vertebral fractures and clinical fractures. These outcomes were obtained from 2174 women who participated in at least one follow-up survey. JPOS is a unique resource of individual-level bone health information with radiological and biological archives that include DXA images, and serum, plasma and DNA for future analyses with emerging radiological and biological techniques. The JPOS dataset is not freely available, but new collaborations are encouraged. Potential collaborators are invited to contact the Secretary General (M.I.) at the administrative office of the JPOS Study Group.

    To determine current practices in the disposal and handling of home medical care (HMC) waste, a questionnaire was mailed to 1965 offices nationwide. Of the office that responded, 1283 offices were analyzed. Offices were classified by management configuration: those attached to hospitals were classified as "attached offices" and others as "independent offices". More nurses from attached offices recovered medical waste from patients' homes than those from independent offices. Most nurses educated their patients on how to store HMC waste in their homes (79.3% of total) and on how to separate HMC waste (76.5% of total). On the other hand, only around half of nurses (47.3% from attached offices and 53.2% from independent offices) educated their patients on where to dispose of their HMC waste. 66.0% of offices replied that patients had separated their waste appropriately. The need for patient education has emerged in recent years, with education for nurses under the diverse conditions of HMC being a key factor in patient education. (C) 2014 Elsevier Ltd.     A questionnaire survey was conducted with the aim of examining the problems involved in the disposal of infectious waste at home-visit nursing stations and in its handling during home visits by nurses. From among the home-visit nursing stations registered with the National Association for Home-Visit Nursing Care, 1,965 offices were selected at random and questionnaires were sent to the selected offices. Nurses at 1,314 offices (66.9 %) responded to the survey and responses from 1,283 offices were identified as suitable for analysis after excluding 26 offices that closed and five offices whose main field of care was psychiatry. Offices were classified by management configuration. Offices attached to hospitals were classified as "attached office" and all others were classified as "independent office". More attached office nurses recovered medical waste from patients' homes than did independent office nurses. They were also more likely to transport waste with them during the course of a day's visits. There was a significant difference between attached and independent offices in the burden of expense for waste disposal. Both offices have strong concern about waste treatment containers and handling in improvement in home medical care (HMC) waste disposal. Thus, in order to alleviate these concerns, it is necessary to provide nurses with containers for medical waste suited to home-visit nursing care and tools for preventing injuries. Japanese government should address HMC waste disposal more comprehensively through necessary legislation, subsidization and standardization.

    Bone development up to early adulthood plays an important role in determining the risk of osteoporosis later in life. However, bone development in children has not been fully documented by longitudinal studies in Japanese children. The purpose of this study is to determine the degree of tracking of areal bone mineral density (aBMD) from pre-puberty to 6-year follow-up, and to determine the target period to achieve maximal peak aBMD. This study was conducted as the pediatric part of a larger cohort study, the Japanese Population-based Osteoporosis (JPOS) study. Of 448 children aged 9-12 years who completed the baseline survey, 225 participated in the follow-up study 6 years later (follow-up rate: 50.2%). aBMD at the forearm was measured using dual-energy X-ray absorptiometry. aBMD values in pre-pubertal children at baseline showed a significant tracking correlation with aBMD obtained at 6-year follow-up in both genders (boys r = 0.655, girls r = 0.759). Although boys and girls in the lowest quartile of aBMD pre-pubertally had greater annual increases in aBMD from pre-puberty to 6-year follow-up than those in other aBMD quartiles, they still showed the lowest mean aBMD at 6-year follow-up. Children with lower pre-pubertal aBMD showed greater increases in BMD up until 6-year follow-up, but the increase was not great enough to catch up with other children. About 50% of the variance in aBMD at 6-year follow-up was determined by the aBMD achieved during the pre-pubertal period. Activities that increase aBMD are important not only for children during puberty, but also for younger pre-pubertal children.

    We analyzed 1,217 women to examine the effect of peroxisome proliferator-activated receptors gamma (PPAR gamma) C161 -> aEuro parts per thousand T on bone status. Among 664 premenopausal women, the C161 -> aEuro parts per thousand T is associated with low bone mineral density (BMD) at the total hip and femoral neck. Moreover, the odds ratio for osteopenia or osteoporosis at the femoral neck was 1.98 for premenopausal CT/TT genotypes. The impact of PPAR gamma on BMD has not been conclusively established. We examined if PPAR gamma C161T polymorphism is associated with BMD and its change. We conducted a baseline survey in 1996 and a 10-year follow-up survey, Japanese Population-based Osteoporosis Study, with a sample population representative of Japanese women. Of these, 1,217 participants in the 1996 survey were analyzed cross-sectionally, while longitudinal analysis was performed on 563 women. A P value < 0.0042 (=0.05/12 for three menstrual statuses and four skeletal sites) was considered statistically significant after Bonferroni correction in multiple testing for cross-sectional analysis. The total hip and femoral neck BMDs were significantly higher for CC genotype than for CT/TT genotypes among 664 premenopausal women (P = 0.0020, P = 0.0022, respectively). Compared to the CC genotype, the odds ratio for osteopenia or osteoporosis (T-scores below -1) at the femoral neck was 1.98 for premenopausal CT/TT genotypes with statistical significance (P = 0.0041). Change of BMD at either skeletal site during the follow-up period was not significantly different for either menstrual status. We conclude that the PPAR gamma C161T is associated with low peak bone mass.

    This cross-sectional study examined the following four variables for impact on adolescent bone growth: the degree of impact, and the period, frequency, and daily duration of physical activity. We studied 127 boys and 136 girls between the ages of 12 and 15 years from northern Japan. Bone mineral density (BMD) at the spine and hip were measured using dual X-ray absorptiometry, and histories of participation in sports club activities beginning in first grade of elementary school were obtained through a questionnaire. The time spent participating in sports club activities between fourth and sixth grades during elementary school (E4-E6) was predictive of increased BMD, adjusted for height, weight, onset of pubic hair appearance, calcium intake, and grip strength, with the exception of hip BMD in females. Analysis of the period, frequency, daily duration of sports club activity, and a score of mechanical impact of physical activity (MECHPA) as substitute for time spent during E4-E6 revealed a significant relationship between the period of activity and BMD, with the exception of spine BMD in females. Activities performed two or more times a week during E4-E6 were also associated with an increased BMD at the hip for males and the spine region for females. Thus, the period and frequency of sports club activity, independent of its degree of impact or daily duration, in the age range of 9 to 12 years may be important for bone growth in children and adolescents.

    We evaluated the value of bone turnover markers, including osteocalcin (OC) and bone-specific alkaline phosphatase in the serum, and type I collagen C-terminal telopeptide and free and total deoxypyridinoline (tDPD) in the urine of fasting patients, in an attempt to predict which osteopenic women [i.e., those with >= 70% and < 80% of the young adult mean (YAM) bone mineral density (BMD)] would progress to the osteoporosis level of BMD (< 70% of YAM). Of the 1153 women without defects in bone metabolism who completed the 3-year follow-up, 147, 161, and 144 women were judged by dual X-ray absorptiometry to be osteopenic from baseline measurements of BMD in the spine (LS), hip (TH), and distal radius (DR), respectively. Progression to the osteoporotic level of BMD was noted for 23.8%, 16.1%, and 12.5% of the subjects with osteopenia of the LS, TH, and DR, respectively, while most of them were in the lower half of the osteopenic level of BMD at baseline. Among the subjects in this lower-level osteopenia category, a significantly higher OC level was observed for the subjects with osteoporosis progression at the LS than those without. The subjects with progression at DR showed a significantly higher tDPD level. The association between OC level and disease progression remained unchanged after adjustments for age, body size, and BMD at baseline. The subjects in the upper one-third category of OC levels showed a 6.4 fold greater risk of progression at LS (95% confidence interval, 1.8-23.1) compared with those in the lower one-third category after the adjustments for age, body size, and BMD at baseline. Receiver operating characteristics analysis showed that the area under the curve was 0.716 for the OC level in the prediction of osteoporosis progression at LS. The levels of OC and tDPD may be useful in predicting which osteopenic women will progress to osteoporosis.

    Japanese fermented soybeans (natto in Japanese), which contain a large amount of menaquinone-7, may help prevent the development of osteoporosis. We assessed the possibility of an association between habitual natto intake and bone mineral density (BMD) and BMD change over time in healthy Japanese women who participated in a large representative cohort study (Japanese Population-based Osteoporosis Study: JPOS study). The BMD was measured at the spine, hip, and forearm in 944 women (20-79 y old) at baseline and at a follow-up conducted 3 y later. Dietary natto intake was assessed by a FFQ on both occasions. Additional covariates including age, height, weight, lifestyle factors, dietary calcium intake, and the intake of other soybean products, were also measured. The total hip BMD at baseline increased (P for trend = 0.0034) with increasing habitual natto intake in the postmenopausal women, although this was not the case at other skeletal sites. There were significant positive associations between natto intake and the rates of changes in BMD at the femoral neck (P < 0.0001) and at the distal third of the radius (P = 0.0002) in the postmenopausal women. The association in the femoral neck persisted even after adjusting for covariates. No significant association was observed between the intake of tofu or other soybean products and the rate of BMD change in the postmenopausal women. Natto intake may help prevent postmenopausal bone loss through the effects of menaquinone 7 or bioavailable isoflavones, which are more abundant in natto than in other soybean products.

    Introduction: The predictive value of biochemical markers of bone turnover for subsequent change in bone density in a population sample of healthy women with a wide range of ages has not been fully established. Methods: We followed 1,283 women aged 15-79 years at baseline selected randomly from the inhabitants of three areas in Japan for 6 years, and examined 1,130 subjects with no disease or administration of drugs affecting bone metabolism. The annual change in bone density at the spine, total hip, and distal one third of the radius was determined during the follow-up period by dual x-ray absorptiometry and was compared among the groups using different levels of biochemical markers at baseline, including serum osteocalcin (OC) and bone-specific alkaline phosphatase (bone ALP), free and total (tDPD) forms of immunoreactive deoxypyridinoline, and type I collagen crosslinked C-terminal telopeptide (CTX) in urine. Results: Premenopausal women aged 45 years or older with elevated levels of OC, bone ALP, CTX, or tDPD showed significantly greater bone loss at most skeletal sites during the follow-up period than those with lower levels, after adjustment for the effects of age, height, weight, dietary calcium intake, regular exercise, and current smoking. The greatest coefficient of determination of the model was observed in the association between CTX and bone loss at the hip during the first 3 years of follow-up (42.8%). These subjects were pooled with perimenopausal women at baseline, and those who still menstruated at follow-up in this pooled group showed significant but more modest associations, whereas those who entered menopause during the follow-up period showed clear associations. However, early postmenopausal women with less than 5 or 10 years since menopause showed an association that was limited mostly to the distal radius, and other postmenopausal groups had virtually no association. Conclusion: Biochemical markers of bone turnover may predict bone loss in women undergoing menopausal transition but may not predict bone loss in postmenopausal women.

    Using a large-scale representative sample of the Japanese female population, we examined the effects of a single nucleotide polymorphism within a binding site of Cdx-2 in the promoter region of the vitamin D receptor gene on bone mineral density (BMD), and the interactions between this polymorphism and lifestyle factors on BMD. Fifty women were randomly selected from each of the 5-year age-stratified populations (15-79 years) in each of three chosen municipalities as a part of the Japanese Population-based Osteoporosis Study. BMD at the lumbar spine, hip, and distal forearm was measured using dual-energy X-ray absorptiometry at baseline and again in a follow-up study conducted 3 years later. Information on lifestyle factors was collected in a questionnaire and followed up in interviews. The G-to-A polymorphism within the Cdx-2 binding site was determined by a TaqMan allelic discrimination assay. At baseline, 1,340 women were analyzed. The baseline BMD in the ultradistal forearm in premenopausal women with the GG genotype was significantly lower than in those with other genotypes. There was no association between the Cdx-2 genotype and the change in BMD at any of the skeletal sites. We found significant associations between daily milk consumption and baseline BMD at some skeletal sites but only in subjects with the GG genotype. In conclusion, the Cdx-2 polymorphism alone did not have a substantial effect on BMD in Japanese women. However, this polymorphism might have some effect in women with low calcium intake.

    The effects of growth and pubertal development on the bone mineral density (BMD) of the lumbar spine and hip in peripubertal Japanese children were studied as a basis for evaluating the effects of modifiable factors on bone mass gain. The study comprised bone mass measurements in the lumbar spine (L2-4), femoral neck, and total hip using dual-energy X-ray absorptiometry as well as body size measurements and detailed interviews on medical history and pubertal status. The subjects were 404 first-grade students in three junior high schools (129 boys and 275 girls, mean age 12.8 +/- 0.3 years) with no diseases or medication that would affect bone metabolism. BMD at each site showed an increasing trend with physical growth and sexual maturity. Significant positive correlations were observed between BMD at every skeletal site and height, weight, and grip strength in pre- and postpubertal boys and girls. In multiple regression analyses, pubertal development had a significant positive independent effect on BMD at every skeletal site in girls, but not in boys. Physical and pubertal development showed major effects on BMD, but the magnitude of these effects differed in boys and girls, even if they were of the same age. We conclude that confounding factors due to physical and pubertal development should be taken into consideration in different ways for boys and girls in investigations on the effects of environmental or behavioral factors on bone mass acquisition in peripubertal children.

    In Japan, the main source of dioxins is incinerators. This study examined the relationship between the distance of schools from municipal waste incineration plants and the prevalence of allergic disorders and general symptoms in Japanese children. Study subjects were 450,807 elementary school children aged 6-12 years who attended 996 public elementary schools in Osaka Prefecture in Japan. Parents of school children completed a questionnaire that included items about illnesses and symptoms in the study child. Distance of each of the public elementary schools from all of the 37 municipal waste incineration plants in Osaka Prefecture was measured using geographical information systems packages. Adjustment was made for grade, socioeconomic status and access to health care per municipality. Decreases in the distance of schools from the nearest municipal waste incineration plant were independently associated with an increased prevalence of wheeze, headache, stomach ache, and fatigue (adjusted odds ratios [95% confidence intervals] for shortest vs. longest distance categories =1.08 [1.01-1.15], 1.05 [1.00-1.11], 1.06 [1.01-1.11], and 1.12 [1.08-1.17], respectively). A positive association with fatigue was pronounced in schools within 4 km of the second nearest municipal waste incineration plant. There was no evident relationship between the distance of schools from such a plant and the prevalence of atopic dermatitis or allergic rhinitis. The findings suggest that proximity of schools to municipal waste incineration plants may be associated with an increased prevalence of wheeze, headache, stomach ache, and fatigue in Japanese children.

    To establish the reference values for quantitative ultrasound (QUS) indices (speed of sound [SOS]), and broadband ultrasonic attenuation [BUA]) in healthy Japanese adolescents, and to evaluate the effects of age and body size on QUS in comparison with their effects on bone mineral density (BMD), 632 healthy adolescents aged 12 through 17 years recruited from a larger cohort study (Japanese Population-based Osteoporosis [JPOS] Study), were examined in terms of bone mass measurements by QUS at the calcaneus (Sahara; Hologic) and by dual-energy X-ray absorptiometry at the distal one-third radius and ultradistal forearm. We present sex- and age-specific mean values of the QUS and BMD indices. BMD increased significantly up to 17 years of age in males and up to 16 years in females. However, the age-related change in the QUS indices in males was not as clear as that seen for BMD and no age-related change in the QUS indices was observed in females. Significant positive correlation coefficients between BMD and body size were observed in both sexes even after adjusting for the effect of age. SOS showed no correlation with body size and BUA showed a positive but weak correlation with body size in both sexes. Thus, the relationships of age and body size to BMD and QUS were different from each other, even though the QUS indices had significant positive correlations with BMD, allowing for the effect of age.

    Background Vitamin D receptor (VDR) gene polymorphisms have been inconsistently associated with bone mineral density (BMD). To precisely evaluate the associations between three VDR gene polymorphisms and BMD, we performed a large-scale representative study of the Japanese female population. Methods Fifty women were randomly selected from each of the 5-year age stratified populations (15-79 years) in each of the three municipalities examined, as a part of the Japanese population-based osteoporosis (JPOS) baseline study in 1996. In the study, BMD at the lumbar spine, hip, and distal forearm were measured using dual energy X-ray absorptiometry (DXA). Two polymorphisms were determined in the VDR gene locus identified by the restriction endonucleases ApaI and TaqI through a novel allele discrimination method using two different allele-specific fluorescence probes. The other VDR gene polymorphism was identified by the restriction endonuclease FokI using the polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) method. Changes in BMD were determined in a follow-up study 3 years after the baseline study. Results After the exclusion of women who had any medical or menstrual history affecting BMD, 1434 women were analysed. The annual per cent changes in BMD at the lumbar spine over 3 years in subjects with tt genotype in the TaqI polymorphism were different from other genotypes, both in the women who were premenopausal at the follow-up survey (F-premenopausal women) and in the women who were postmenopausal at the baseline survey (B-postmenopausal women). However, the effects of tt genotype on change in BMD were opposite in the two groups. In addition, these associations or tendencies were not observed at the different skeletal sites. Conclusions This study revealed that none of the individual VDR gene polymorphisms displayed consistent association with baseline BMD or BMD change. Therefore, the effect of the VDR genotype on bone mass is negligible in Japanese women.

    To assess the dependence of calcaneal quantitative ultrasound (QUS) indices (speed of sound [SOS] and broadband ultrasound attenuation [BUA]) on foot temperature and season, we measured these indices monthly, from August to February of the next year, in five healthy Japanese adults (three men and two women, aged 31-52 years), using a dry-type QUS device (Sahara; Hologic). The monthly QUS measurement was performed three consecutive times after the start, at 30, 60, and 120 min from the start, in a test room where the temperature was kept at 25 +/- 2degreesC. Skin temperature was monitored at the dorsum pedis throughout the measurements. A significant seasonal trend in the monthly mean Z-scores of QUS indices was observed (P < 0.001). QUS indices were higher during the winter than summer, while skin temperatures were lower during the winter. SOS showed a significant inverse association with skin temperature (r = -0.64), while BUA did not (r = 0.09). Skin temperature at the dorsum pedis slowly declined and SOS increased during the experiment. BUA decreased by 60 min from the start of the experiment and then increased slightly. The coefficient of variation (CV) of both QUS indices showed the smallest values at 60 min from the start. In conclusion, there are seasonal variations in the QUS indices, and SOS particularly depends on skin temperature. When a subject is followed up for years, the QUS measurements should be conducted in the same season. In a setting of community-based medical checkups using QUS devices, health professionals should measure the QUS indices of a subject after 60 min of rest in the examination room.

    To establish the reference values of the quantitative ultrasound (QUS) indices in healthy Japanese women and to propose a diagnostic criterion for osteoporosis by means of the QUS indices, 659 healthy women aged 20-79 years recruited from a larger cohort study (JPOS study), were examined for bone mass measurements by QUS at the calcaneus (SAHARA, Hologic Inc., USA) and by dual-energy X-ray absorptiometry at the spine, hip, and distal forearm. We presented 10-year age-specific mean values and T-scores of the QUS indices. The pattern of decrease in the T-score appeared to be linear in the QUS indices and total hip BMD but not in BMD at the spine. The T-score of the QUS of indices of the subjects in their 70s were significantly higher than that of BMD at the spine. The prevalence rates of osteoporosis in the subjects aged 50 and older diagnosed by QUS (8.7% for SOS, 10.7% for BUA) were similar to that diagnosed by total hip BMD (11.5%) and significantly lower than that by the spine BMD (36.1%) when the WHO criteria were applied. We performed receiver-operating characteristic analysis to set a cutoff level of the QUS indices for the diagnosis of osteoporosis to accurately identify the subjects diagnosed by either the spine or total hip BMD. The highest likelihood ratios for SOS and BUA were obtained at the cutoff levels of 1,517.7 m/sec (T-score: -1.58) with the sensitivity of 0.65 and the specificity of 0.65 and 59.5 dB/MHz (T-score: -1.52) with 0.66 and 0.69, respectively. The diagnostic accuracy of QUS indices for osteoporosis was not superior to that of age. However, the QUS indices showed a significant contribution to forming the diagnosis of osteoporosis independently of age and body size in multivariate diagnostic models developed by the logistic regression analysis. Therefore, the cutoff values presented in this study may be used as a tentative criterion until the cutoff levels for the QUS indices are set according to the fracture risk.

    高齢化の進行や生活習慣病の低年齢化に伴い、在宅医療の必要な人は増え続けています。在宅医療廃棄物は周知のように法では一般廃棄物に該当することから、市町村が処理責任を負っています。しかし現状は、ほとんどの市町村が在宅医療廃棄物のうち注射針を受け入れていないほか、それ以外の通常感染性を有さないと考えられるビニールバッグ類等についても、感染性の可能性が皆無ではない、感染性が無いことが確認できない等の理由により受け入れられていないケースが見受けられます。市町村が受け入れない廃棄物については、現時点では医療機関や薬局等の医療関係者が回収しています。将来的に医療と高齢化が進行すると、在宅医療患者数はさらに増加し、通院できないケースも考えられます。そういった状況が来る前に在宅医療廃棄物の処理ルートを確保する必要があると考えられます。この研究では、比較的在宅患者が多いと思われる政令指定都市、中核市を中心に在

    本研究は、留学に向ける「失敗不安」の構造を解明し、効果的な留学体験を実現させる方策の構築を目的とする。また、物理的な移動が制限される現在、国内に居ながら心理的なシミュレーションを通して留学での学びの準備を行うことも目的の一環とする。本研究では、近日中に留学に赴く人、この先留学を希望する人、留学を希望しない人の意識調査を、次の3つの調査を用いて比較分析する。そのために次の3種類の調査を行う。 1)BEVI検定:欧米・アジア諸国で広く用いられる国際検定で、学習または変化を促進する目的で作られた教育プログラムの影響・効果を分析する。 2)スチューデントEQ: 日本の大学生向けの自己発見・自己診断ツールであり、情意の知能指数と呼ばれるEQ(Emotional Quotient)に由来する行動変容を分析する。 3)Googleフォームによるインタビュー: 不安に感じる(感じた)ことの経験、内容、対処、解決の有無などを自由記述で回答してもらう。 ・2021年度は、一学部のみの限定的な調査になった。最終的に約10名から調査協力の同意を得て、3月末に調査を締め切り、4月現在、分析中である。

    研究成果の概要:骨粗鬆症性骨折のリスク評価法を確立するため、JPOS Cohort 1515人を満10年追跡した。DXAによるdigital写真で診断した新規椎体骨折のリスクを多重logistic回帰で評価すると、既存椎体骨折、年齢、大腿骨近位部骨密度、閉経、骨折既往・卵巣摘除・高血圧・高脂血症・骨粗鬆症の既往を独立変数としたモデルでROC曲線下面積が0.752となったが、実用化にはもう一段の改善が必要と考えられた。

    <対象と方法>日本国内にある3市町(福島県西会津町、香川県さぬき市、沖縄県宮古島市)において、平成11年に実施された調査を受診した成人女性1588人のうち平成14年度フォローアップ調査を受診している1173人を対象に、腰椎及び大腿骨頸部の骨密度測定、超音波による踵骨骨量測定、身長・体重測定、問診による骨代謝に影響する既往歴や薬剤の服用歴、運動歴、乳製品の摂取等の食習慣や運動習慣などを一定の様式により保健師が聴取。食品摂取頻度調査票によるカルシウム摂取量調査を行った。骨折の判定はMcCloskey-Kanisの椎体変形判定法による診断基準により行った。調査データに欠損のない対象者のうち、骨代謝に影響する既往歴や薬剤の服用歴がなく、ベースライン時45歳以上の閉経者200人を対象に腰椎BMD、踵骨超音波指標(SOS,BUA)による骨折の予測を行った。腰椎BMDはWHOの骨粗鬆症判定基準である若年成人者平均-2.5SD未満を、踵骨SOS、BUAの判定基準は我々が以前に論文化しており、SOSは若年成人者平均-1.58SD未満を、BUAは若年成人者平均-1.51SD未満を骨粗鬆症とし、ベースライン時に骨粗鬆症と判定されるものがフォローアップ時の骨折をどの程度予測するかを検定した。 <結果>腰椎BMDによる骨折予測では感度0.391、特異度0.638。踵骨SOSでは感度0.652、特異度0.627。踵骨BUAでは感度0.696、特異度0.554でSOS、BUAとも腰椎BMDよりも骨折予測成績が優秀であった。またベースライン時に腰椎BMDで骨粗鬆症と診断された者がフォローアップ時に骨折するオッズ比は1.12。それに対しSOSでは同様に2.76、BUAでは2.53と踵骨超音波指標は将来の骨折予測に有用であることが判明した。

    Using a large-scale representative sample of the Japanese female population, we examined the effects of single nucleotide polymorphisms in several candidate genes on bone mineral density (BMD), and the interactions between these polymorphisms and lifestyle factors on BMD. For 1,651 women aged 15 to 79 years at baseline selected randomly from 3 municipalities in Japan, we measured BMD at the lumbar spine, hip and distal forearm using DXA at baseline, and again in a follow-up study conducted 3, 6 and 10 years after the baseline as a part of JPOS Cohort Study. Among the cohort from which 133 women dropped out during the 10-year follow-up because of deaths and move of residence, 1040 completed the study. Vertebral fractures were diagnosed using lateral chest radiographs with vertebral morphometry in 720 women aged 50 and over Clinical fractures were assessed by the interview. The changes in BMD varied according to ages of subjects. BMD did not show any significant change until 45 years at every skeletal site. After that, an abrupt increase in bone loss occurred at each site. The polymorphisms in the VDR, TGF-beta, Osteocalcin, MTHFR, CYP19, CYP1A1, COMT, PPARγ, RUNX2, HSD17beta and BMP2 gene were determined by the PCR-RFLP method and a TaqMan allelic discrimination assay. At baseline, 1,340 women were analyzed. There were no significant associations between each genotype and the baseline BMD or the change in BMD during the first 3 years of follow-up at any of the skeletal sites. We found significant interactions between the natto intake and CYP19 and COMT polymorphisms on BMD at femoral neck in postmenopausal women. In addition, the prevalence of osteoporosis and osteopenia was different between the genotypes of CYP19. Currently, we are studying the association between these candidate genes and incident fractures during 10 years follow-up.

    平成16年度に行ったフォローアップ調査のデータをまとめ、学会発表(17^ IEA World Congress of Epidemiology)を行った。以下にその概要を示す。 成長期における踵骨超音波指標の変化の様相を5年間追跡のデータを用いて第二次性徴の発来時期別にグループ分けし、分析した。踵骨超音波指標(SOS, BUA)腰椎、大腿骨近位部BMDは345人の男女(男子126人、女子219人)から得られた。ベースライン時の測定値とフォローアップ時の測定値から変化率を求めた。対象者は第二次性徴の発来時期別に4群に分けられ、解析に用いた。腰椎、大腿骨近位部全体BMDは男女とも第二次性徴の発来時期が早い群ほど有意に増加していた。しかしながら、この関係は男子において身長、体重を調整すると有意ではなくなった。踵骨超音波指標は女子のBUAを除いて、このような関係は見られなかった。踵骨超音波指標の変化率は男子では第二次性徴発来時期の最も早い群において有意に大きかった。しかしながらBMDの変化率では第二次性徴発来時期別の有意な関係は何も見られなかった。一方、女子ではBMDの変化率は第二次性徴の発来時期に伴って有意に低下したが、踵骨超音波指標は第二次性徴の発来時期別の変化は見られなかった。 第二次性徴発来時期別の踵骨超音波指標変化率はBMDで観察されたそれとは異なっていた。BMDは面積密度を測定するので、骨の大きさが大きくなることによりその数値が上昇してしまうことにより骨量の実際の変化を過大評価しているだろう。踵骨超音波指標は第二次性徴期のような成長速度の速い時期における実際の骨量増加を評価するうえで、BMDよりも有益な情報を与えることがわかった。

    Incidence of fracture in Japanese children has increased progressively in these decades. Investigations into risk factors for children's fracture may lead to prevention of fractures later in life. We examined possible causes of fractures in growing children by estimating volumetric bone mineral density (vBMD) in a longitudinal study. We examined healthy 570 children (283 boys) from 4th (10 or 11 years) to 9th grade of a compulsory school (G4 to G9, respectively) for whom areal BMD (aBMD) at the spine (LS) and total hip (TH) were measured by DXA. The subjects were invited for a follow-up survey 3 years after the baseline. 432 subjects (203 boys) (75.8%) completed the surveys. vBMD was estimated at LS under an assumption of the spine being a cylinder. Height and aBMD at LS and TH at baseline increased linearly with advancing in age in both genders. Similar increase was observed in vBMD in girls but not in boys, who showed the lowest vBMD in G6 (12 or 13 years). The greatest annual rate of changes in height and aBMD were observed during G6 to G7 in boys and G4 to G5 in girls. However, the annual rate of change in vBMD was the greatest during G8 to G9 in boys and during G7 to G8 in girls. The rates of change in BMDs were greater in those who had heavier weight, greater intake of milk and greater physical activities. The lowest vBMD was observed at the age of peak velocity of height growth which is the age at the highest fracture incidence in both genders. Delay in gaining bone density compared with height growth may cause low bone density, which may result in the transient increase in fractures in growing children, and may be prevented to improve the lifestyle factors found in this study.

    Using a large-scale representative sample of the Japanese female population, we examined the effects of single nucleotide polymorphisms in the VDR, TGF-β, Osteocalcin, MTHFR, CYP19, CYP1A1 and COMT gene on bone mineral density (BMD), and the interactions between these polymorphisms and lifestyle factors on BMD. Fifty women were randomly selected from each of the 5-year age-stratified populations (15-79 years) in each of three chosen municipalities as a part of the Japanese population-based osteoporosis (JPOS) baseline study. BMD at the lumbar spine, hip and distal forearm were measured using DXA at baseline, and again in a follow-up study conducted three years after the baseline. Information on lifestyle factors was collected in a questionnaire and followed up in interviews. The polymorphisms were determined by the PCR-RFLP method and a TaqMan allelic discrimination assay. At baseline, 1,340 women were analyzed. There was no association between each genotype and the baseline BMD or the change in BMD at any of the skeletal sites. We found significant associations between daily milk consumption and baseline BMD at some skeletal sites, but only in subjects with GG genotype within a binding site of Cdx-2 in the VDR gene. There were significant positive associations between the natto intake and the rates of changes in BMD in postmenopausal women of JPOS study. We found significant interactions between the natto intake and CYP19 and COMT polymorphisms on BMD at femoral neck in postmenopausal women. In addition, the prevalence of osteoporosis and osteopenia was different between the genotypes of CYP19. In conclusion, each polymorphism alone did not have substantial effect on BMD in Japanese women. However, these polymorphisms might have some effect in women with specific lifestyle factors. Currently, we are studying the effects of other candidate genes on BMD and searching new polymorphisms associated with bone formation.
